The study led by Dr. Hance Clarke, Director of Pain Services at University Health Network (UHN), and has been published in the peer-reviewed Canadian Journal of Pain.

The prospective, observational study evaluated patient-reported outcomes among individuals with chronic pain receiving physician-authorized medical cannabis in routine clinical practice. The study reflects a health-care provider-supervised model of medical cannabis use and was supported by multiple clinicians and medical institutions across Canada. The study was led by Dr. Hance Clarke at the University Health Network (UHN) and supported by several other clinicians and medical institutions.  

“While the term ‘medical cannabis’ is widely used, this study specifically evaluated outcomes among patients using medical cannabis under the supervision of a health-care provider,” said Dr. Hance Clarke, Principal Investigator and Clinician at UHN. “Among chronic pain patients who remained on an authorized medical cannabis product at six months, improvements were observed in pain severity, pain interference, and several health-related outcomes. These findings support the potential role of medical cannabis in selected patients as indicated by preliminary real-world data, while highlighting the importance of clinician involvement in monitoring efficacy, safety, and ongoing patient outcomes”.

Study Results Summary

The publication reports findings from a cohort of chronic pain patients enrolled in a prospective, observational study assessing patient-reported outcomes over a 24-week follow-up period. Outcomes were measured using validated assessment tools commonly employed in pain and mental health research.

The observational results demonstrated statistically significant improvements from baseline in measures of pain interference and pain intensity, as well as improvements in validated measures of anxiety, depressive symptoms, and overall quality of life over the 24-week observation period. While the magnitude of change did not meet established minimal clinically important difference thresholds, the observed directional improvements are consistent with findings reported in existing real-world and clinical literature. The results underscore the need for further placebo-controlled randomized trials to better define optimal dosing strategies, formulations, and routes of administration for medical cannabis.

Study Background and Ongoing Enrollment

The study was implemented through the MyMedi.ca platform and Avicanna provided patient support services, and clinical and data infrastructure required for large-scale real-world evidence research. The MCRWE study is ongoing, with continued patient recruitment under the leadership of Dr. Clarke. Participants have been enrolled across seven Canadian provinces, with support from multiple medical institutions. The study is designed to evaluate patient-reported outcomes associated with medical cannabis use, including pain, sleep, anxiety, depression, and epilepsy, using validated assessment tools over a 24-week follow-up period. (“Avicanna” or the “Company”) (TSX: AVCN) (OTCQX: AVCNF) (FSE: 0NN), a biopharmaceutical company focused on the development, manufacturing, and commercialization of plant-derived cannabinoid-based pharmaceuticals, today announced positive preclinical results and the filing of a provisional patent application with the United States Patent and Trademark Office (USPTO) for its novel Powder Drug Delivery System (“PwdRx”) platform.

The drug delivery platform has been specifically developed to address key formulation challenges associated with highly lipophilic cannabinoids (CBD, CBG, THC and CBN), which often exhibit poor water solubility, low and variable bioavailability, and delayed onset of action. In preclinical pharmacokinetic studies, the PwdRx formulations demonstrated enhanced bioavailability and faster uptake compared to conventional medium-chain triglyceride (MCT) oil-based formulations.

Recent in-vitro study of Avicanna’s PwdRx technology demonstrated statistically significant results including 74% higher bioavailability (AUC), 63% faster peak plasma levels (TMAX) and a 134% higher peak plasma concentration (CMAX) when compared to MCT oil formulation.

Management Commentary:

Q2 2025 Financial Highlights:

  • Revenue: The Company generated revenue of $6.40 million and $18.88 million for the three- and nine-month periods ended September 30, 2025. Canadian revenue increased by 4%, marking a second consecutive quarter of revenue growth across Canadian business units. This is a direct result of increased investment in medical affairs initiatives. International revenue also increased by approximately 6% as compared to the second quarter of 2025.
  • Gross Profit and Margin: Gross profit of $3.15 million and $9.88 million for the three and nine-month periods. Resulting in gross margins of 49% and 52%, respectively. This represents a short-term reduction in gross margin from 57% for the prior year three-month period and is due to non-recurring costs, the Canada Post strike and one-time adjustments. The nine-month margin represents a slight improvement from 50% in the prior period and was largely a result of sales mix changes from product sales to licensing and service revenue.
  • Adjusted EBITDA: The Company reported an adjusted EBITDA loss of $0.79 million which marks a decrease over the respective three-month period in 2024. The nine-month adjusted EBITDA loss was $0.61 million, which marks a slight improvement over the nine-month period loss of $0.72 million during 2024. The short-term reduction in adjusted EBITDA is largely a result of the reduction in gross margins attributed to non-recurring factors and one-time corrections.

Other Q3 2025 Corporate Highlights:

  • Canadian commercial advancements: Canadian commercial advancements in the third quarter across all business units resulted in record sale of 62,987 units, representing a 39% increase compared to the same period in 2024. At the end of the third quarter the Company had 52 commercial SKU’s and 174 commercial listings across medical and adult use channels, representing 29% growth in total listings from Q3 2024, and 18% growth from Q2 2025. The MyMedi.ca platform delivered its second consecutive quarter of growth driven by Medical Affairs efforts. Other Canadian business units including B2B Medical achieved a 113% increase in products sold. In addition, the Adult-Use business unit entered into three new provinces and territories including Newfoundland, Yukon and the Northwest Territories.

Q2 2025 Financial Highlights:

• Revenue: The Company generated revenue of $6.16 million and $12.48 million for the three- and six-month periods ended June 30, 2025, representing a 1% increase and a 1% decrease, respectively, compared to the corresponding periods in 2024.

• Gross Profit and Margin: Gross profit for the second quarter was $3.13 million, with a six-month total of $6.73 million, translating to gross margins of 51% and 54%, respectively—up from 47% in the prior-year periods. This improvement was primarily driven by increased service and licensing revenue from international markets.

• Adjusted EBITDA: For the second quarter, the Company reported an adjusted EBITDA loss of $0.25 million and an adjusted EBITDA gain of $0.18 million for the six-month period. This marks a notable improvement from the same periods in 2024, when adjusted EBITDA losses were $0.44 million and $0.42 million, respectively. The improvement reflects both reduced operating expenses and enhanced gross margins.

Other Q2 2025 Corporate Highlights:

• Canadian commercial advancements: The Company completed the second quarter with 50 commercial SKUs and 147 commercial listings representing a 19% increase in total SKUs and a 9% growth in total listings from Q1 2025. The Company sold 50,789 units during the second quarter, a 21% increase against the comparable period in 2024.

• Avicanna Announced Sponsorship of Pilot Phase II Clinical Study on Osteoarthritis Pain: The multicenter, blinded, randomized placebo-controlled investigator-initiated study is led by Dr. Hance Clarke and conducted at University Health Network. The study is to analyze effectiveness of Avicanna’s proprietary oral cannabis extracts for osteoarthritic pain and is Avicanna’s first placebo controlled, blinded randomized multicenter trial (“RCT”). The study is funded by a Canadian Institutes of Health Research grant and sponsored by Avicanna, which is providing its proprietary CBD and THC capsules produced under Good Manufacturing Practices for the trial.
